Sales Tax Form |
Under
California State Law, Show Management is
required to keep a record of each exhibitor’s
sales tax information for inspection upon demand
by the State.
ALL EXHIBITORS must complete the
RoboNexus Sales Tax Form and return via fax to
Debbie Camerato by 9/19/05. |

Mandatory Proof of
Insurance Certificate |
Please note
that pursuant to paragraph 5 of the exhibitor
contract "Exhibitor shall, at its expense,
obtain from reputable insurance companies (i)
liability and property damage insurance in an
amount no less than $1 million per occurrence,
and (ii) workers’ compensation insurance
covering its employees in at least the statutory
amount, and shall provide evidence of such
insurance to RT."
Please request a "certificate of insurance in
the name of Robotics Trends" from your insurance
company. This is a one page certificate. Please
submit the certificate via Email to
Dan Kara or FAX at (508) 351-6558.
This mandatory Insurance
Certificate must be submitted by September 30.

Frequency Management Form |
Given the
number of robots and robotics technology being
demonstrated at RoboNexus, we must do what we
can to minimize the chance frequency conflicts.
Please fill out this form to the best of your
ability and Email it back to
Dan Kara
by 9/2/05.

RoboNexus EAC Packet |
If an exhibitor plans to use a
vendor other than the “Official Show Vendor”,
please complete the RoboNexus EAC packet and
return it to Debbie Camerato by 9/23/05. This
packet contains the Exhibitor EAC form, the
Contractor EAC form and EAC Insurance
Certificate. |
